Welcome to the Collin County Clerk's vital records online ordering system for certified copies of birth certificates, death certificates, and marriage certificates. Online requests will be processed during normal business hours, 8:00 AM to 4:00 PM, Monday through Friday, excluding holidays.
This is an official request for a Vital Record from the Collin County Clerk's Office. Vital records include birth, death, and marriages for Collin County. The information contained in this request should be considered private. Please complete all information in full and then finalize the order process through payment of the 'order fee'.
Please note the following:
- Birth and Death records are not public record. Qualified Applicant status is required to obtain a certified copy of a birth or death certificate.
- NON-REFUNDABLE INFO: Applications submitted by a non-qualified applicant will be denied. If your application has been Denied, you will be notified by email. The Service Fee, and any related credit card fees are considered NON-REFUNDABLE. The remaining Payment Amount (certificate fees & shipping fees) will be automatically refunded back to the applicant's credit card or bank account. Make certain you are a qualified applicant prior to submitting the application in order to avoid any denied requests and service fees.
- The Collin County Clerk’s Office maintains long form birth certificates for individuals born within the Collin County registration district.
- This registration district does not include the Methodist Richardson Hospital. These records are filed in Dallas County.
- If the birth occurred outside of Collin County, but within the State of Texas, you will be issued an Abstract Record (short form), which is still a certified legal document.
- If the birth did not occur within the State of Texas we are not able to access your birth certificate. You will need to contact the state where the birth occurred.
- Death Certificates are available only for decedents who pass within Collin County (excluding Methodist Richardson Hospital).
- Marriage Certificates: Collin County can only issue certified copies of your marriage license if you purchased your license with Collin County. If you had a ceremony in Collin County, but you purchased your license in a different county, you will need to contact the county of purchase in order to request a certified copy.
Attention: The maximum number of Birth Certificates that can be ordered per person, per lifetime is TEN (10).
Processing of Applications:
- You will receive an email notification when your application has successfully been submitted into the online ordering system.
- You will receive a second email notification if our office has notified you of needed documents or questions regarding your order.
- Once your application has been approved, a confirmation email will be sent to notify you that your document is ready for pick-up, or will be mailed or shipped by the following business day. The document can ONLY be picked-up by the applicant that places the order.

- If your application has been denied, you will be notified by email. The service fee and any related credit card fees are considered NON-REFUNDABLE. The shipping fees and additional certificate fees will be refunded back to the applicant's credit card or bank account.
* Warning: The penalty for knowingly making a false statement in this form can be 2-10 years in prison and a fine up to $10,000 (Health and Safety Code 195.003)
Attention:
The Texas Department of State Health Services requires that all applicants upload a form of identification when submitting their application. See the list below for information on the forms of identification acceptable. When applying online, we only accept PRIMARY forms of identification (Group A).

Please provide ONE (1) from GROUP A:
- Most Common Group A IDs:
- Driver's license from a U.S. state
- Federal or state ID card
- Military ID card
- U.S. passport
Other Acceptable Group A IDs:
- License to Carry a Handgun
- Pilot's license
- Law enforcement employment ID (federal, state, or city)
- Offender ID issued by the Texas Department of Criminal Justice or an ID from a federal or U.S. state correctional facility or institution
- Department of Homeland Security, United States Citizenship and Immigration Services (USCIS) issued:
- Employment Authorization Document (EAD)
- Permanent Resident Card (Green Card)
- Travel documents:
- Re-entry permit
- Refugee travel document
- Advance parole
- SENTRI card
- U.S. citizen ID card
- U.S. Department of State issued:
- Border Crossing Card (BCC) – B1 for business or pleasure or B2 medical purposes
- Visa

You will be notified by us once we have processed your application and the certificate(s) are out for mail, shipped, or ready for pick-up.
